Where the hell...

do you get the V55/4 form and V355/4 booklet from to apply for SVA?

Tried seracing the DVLA website, but they barely make mention of them (apart from a pamphlet that tells you to fill the form in and refer to the booklet for guidance on how to do so)....

Bloody DVLA are the most frustrating organisation I've ever come across

You can apply for SVA using the SVA form but for registration you need forms from the DVLA that need filling out in triplicate. You need to ring them and ask for them....

Go to your local DVLA office. You can only get them there as they are numbered sequentially and recorded against your details.

I sent my enquiry in to the local office here for the chassis number and they replied with the V55/5 and all the other paperwork.

You only need SVA1 from the website to apply for SVA but don't forget to send in a photo diary of the build with it.
