SVA/MAC - what, where & when?

This must sound like a niave question but can someone please give me a blow by blow account as to how to apply for SVA.
I've been on the VOSA website and downloaded SVA application but it doesn't say who it should be sent to and whether I need an MAC before applying.
I've made up my own VIN as it seems impossible to apply without one (don't want to involve the local office as that sounds like a whole can of worms).
Also if the engine/gearbox/ axle etc are from old vehicles do I still say in answer to question 11 that it was made this year (I'm assuming it will have a Q plate as they aren't all the same donor)?

post it to the address on the first page of the form
Your application will be delayed if you do not fill in this form accurately or in full. When you have filled it in,
please send it, together with your cheque or postal order to SVA Section, VOSA, 91-92 The Strand, Swansea,
SA1 2DH. Please make your cheque or postal order, payable to ‘VOSA’ and check the fee list on the back.

The MAC is the certificate you get when your car passes (ministers approval certificate) and check the latest prices for SVA as i think they went up a week or 2 ago

I thought about using Paul Jepson but in the end did myself and saved the £39, it's not that difficult and there's plenty of help on here.

Do a bit of a search in the SVA/Legal forum, and look at the sticky thread at the top (Registration and SVA) for some more clues.

There is a distinction between:

VOSA, who do the SVA to check if the car is built OK. These guys are usually fairly straightforward to deal with.

DVLA which issues registration numbers/documents... each local office is a LVLO (local Vehicle Licensing Office). These are the chappies who seem to vary from town to town...

P.S. - yes, year of manufacture is when you built it, not when the donor bits were made.

Scary stuff finally admitting it's finished - suppose I'll now cop for all those domestic projects that have been on hold!!

Actually had a phone call from a nice lady in Swansea.
I'd filled in the wrong form (doh!) but if I downloaded the correct one and faxed it over she'd sort it out for me - some of them are human after all.
Now just waiting, waiting.......
