How to make adjustable top bones?

I fancy making some adjustable top bones but not quite sure how they work.
Is it just a longer thread on the ball joint with a nut either side?

Same top bulljoint (well mine are) then ive a gts sleeve that fits from inside of bone so undo locknut and adjust sleeve
