I think it's a trick, no way would you remain that calm after a botch of that magnitude.
The position with the knife would have been marked (on the bottom of the 'special cup' probably), so it would be hard to get wrong.

Just noticed at the end he moved the table by hand. Presumably because that was the 'special' cup 
Come on people, he always keeps the back of his right hand away from view....
Fake. There was blood surrounding the wound instantly - if it were real it would either have seeped out gently, or pissed out all over the place if
he'd hit an artery.
Plus his reflex action would have been to pull his hand back straight away.
Did make me cringe though. 
You can see the fake knife sticking out of the back of his hand at various points in the clip... mainly when silhouetted against the fireplace!
