ebay do them as mentioned above. Honda civic circa 92-2000

ive just got a 2nd hand one ,looks like new.for 40 quid myself. nice and easy to mount too.

Good value at £95 (including the p&p), but I'd make a bracket for the fan rather than using those horrible "tie wrap" through the core fittings.
